Who Needs to Register?

Each parent, guardian, or court-involved party must register separately before services can begin.

What’s Included in Registration:

Completed Intake Packet
Personal Information
Legal or Custody Documentation
Emergency Contacts

Copy of Court Order
If applicable, a current court order outlining visitation arrangements or supervised contact must be submitted.

Photo ID
Government-issued ID for identity verification

Non-Refundable Registration Fee
A one-time administrative fee (amount specified in intake packet)
Payment required to schedule intake session

Consent & Agreement Forms
Program policies
Code of conduct
Waivers and confidentiality agreements

Program Orientation Appointment
Scheduled once the above steps are completed
Required before any supervised visit can take place
